Notes
Service notes, shift documentation and incident reports with signature and co-signature, and a void and amend history that is never rewritten.

Who uses it
- Staff
- Direct support staff, teachers and therapists write, tag and sign notes. Supervisors co-sign another person's note, and administrators void with a written reason. Any staff member who writes notes can correct a signed note with an amendment.
- Families
- Families do not use Notes. It is staff documentation, and it never appears in Assure.
Where it appears
- Assure Staff on the web
- Writing, signing, co-signing, amending and reviewing notes, and the notes reports.
- Assure Staff
- A quick draft at the end of a shift or a visit, and signing your own drafts, on a phone.
Documentation that holds up
Service notes are billing evidence. Notes carries the fields payers audit: the service code, the time window, the location, the signature, and a supervisor’s co-signature. Incident reports are a kind of note, under the same rules.
Signed means signed
A signed note cannot be edited. Corrections are made by writing an amendment that replaces the original in the record; the original keeps its text and its signature. A note can be voided with a reason, never deleted. The history shows every version, who wrote it and when, and why a voided note was voided. Documentation with a rewritable history is not documentation.
Tags, and what they add up to
Notes can be tagged, for behavior, health, injury, medication, mood, sleep, eating, social or an achievement. The notes reports count them by day, by kind and by hour of the week, so a pattern like incidents clustering on Monday afternoons shows up without anyone reading every note. The reports count; they never open a note’s text.
Encrypted per record
Clinical narrative is sealed under its own encryption key before it reaches the database, with that key wrapped by AWS KMS. A single key does not unlock the rest, and a database dump is not a disclosure.
Works on its own or alongside what you already use
Every module sits on the same core: your roster, the adults authorized for each person and what each may do, their approved photos, your locations, and your staff accounts. Add another module later and it uses that same record. How the platform fits together
Other modules
- Handoff Pickup and departure: adults approved in advance, a live queue, a pickup code, and a photo or ID check before anyone is released.
- Messages Conversations between staff and families, tied to the person in care, so the context travels with the message.
- Progress Goals, observations and progress reports built from the data, not from recollection.
- Schedules Staff shifts, availability and time off, groups, the calendar and ways home, with conflicts flagged before they reach the floor.
Start with Notes, or add it to what you run
Tell us which module you need first. We switch modules on for your organization, so adding another later is a setting, not a migration.
